| The
scholarship award is a one time payment of $500.00. A "student"
is defined as a dependent child, stepchild, or ward of a member of Division
169, PEF/Encon, on the date of the drawing. Students are eligible for this
scholarship only if they are enrolled in a full-time degree program (12+
credit hours or equivalent) during the Fall 2004 academic year. Proof of
enrollment will be requested from the winner. The award will be given to the
student in the student’s name.
Two winners will be drawn at random from a hat at the 26th Annual PEF Convention, October 17 - 20, 2004. Applications MUST be received no later than October 8, 2004 to be eligible.
Please complete this form legibly, and return it to any PEF/Encon Steward or forward it directly to: Terry Tyoe, PEF/Encon Secretary NYSDEC, State Office Building Rm 1404 207 Genesee Street, Utica, NY 13501. PEF/Encon will accept only one application per student per member. You may include more than one application per envelope. Please Note: A student is eligible to win one (1) $500 award per drawing.
